Summary

The Human Trafficking Patient Navigator will provide intake, case management and coordination of services to adult and child victims of human trafficking encountered within the healthcare setting.

This position is a full-time, 40-hour workweek (Monday through Friday), some evening and weekend work maybe be required. Travel will be required for some activities and is included in the base salary. All other benefits provided to Coastal Family Health Center personnel are included and will be funded through the SOAR Demonstration Program.

This position will be assigned to the Coastal Family Health Center, Behavioral Health Unit, and is funded by the U.S. Department of Health and Human Services, Administration for Children and Families, Office on Trafficking in Persons, under the SOARing in Mississippi Demonstration Program (timeframe of award is 9/30/22 until 9/29/27). The purpose of this program is to establish a continuum of care and implement human trafficking response protocols throughout the lower nine counties of Mississippi.

Employment will be contingent upon funding and agency requirements.

  • General physical requirements include but are not limited to bending, stooping, climbing stairs, prolonged sitting of more than 4 hours. Requirements

    Qualifications/Education Requirements

    Minimum Qualifications:

    Bachelor's degree from an accredited college or university in a social science such as social work, psychology, public health, or other related area. Experience working in behavioral health, mental health, child welfare, or other related profession is required. The salary for the position with these requirements will be $43,000.00.

    OR

    Master's degree from an accredited college or university in a social science such as social work, psychology, public health, or other related area. Experience working in behavioral health, mental health, child welfare, or other related profession is required. The salary for the position with these requirements will be $45,000.00.

    Preferred Qualifications:

    Applicants with a Master's degree that are bilingual (e.g., Spanish, Mandarin, Vietnamese) and trained in crisis intervention are preferred.

    Core Competencies
    • Assess the needs of patients and provide services under the SOAR Demonstration Program and Coastal Family Health Center.
    • Provide direct services such as intake, navigation, education, and follow up services for those identified as having current or former trafficking experiences.
    • Assist in data collection and reporting of data indicators required by the SOAR Demonstration Program.
    • Coordinate medical services with clinic and behavioral health personnel.
    • Assist in community outreach, universal education, screening, safety planning, and risk reduction.
    • Participate in joint activities involving dissemination of information regarding the SOAR
    • Demonstration Program and Coastal Family Health Center.
    • Collaborate with community partners and social service agencies to obtain services for patients and make referrals.
    • Participate in required trainings and support services for the SOAR Demonstration Program.
    • Maintain compliance with Coastal Family Health Center's policies and confidentiality related to patient information.
    • Performs other miscellaneous duties as assigned. Agency Compliance
      • Maintain comprehensive and organized files utilizing the automated data and case management system.
      • Adhere to the agency's confidentiality policies, maintaining a high-level standard of confidentiality.
      • Compile and accurately report all applicable program statistics and educational activities on time and as directed.
      • Report any known or suspected child abuse to the Mississippi Department of Child Protection Services Centralized Intake.
      • Report any suspicious activity or persons to the immediate supervisor or 911.
      • Attend and participate in agency staff meetings and training.
      • Report to work on time as scheduled.
      • Work with other program components and staff to communicate agency and client concerns, recommendations, and needs effectively.
      • Perform all other duties and/or participate in special projects as assigned by the direct supervisor or Chief Executive Officer.
